Older persons support team
The older person support team supports and promotes the quality of care provided for people who live in residential and nursing homes across North Tyneside.
In the team we have a:
- senior physiotherapist
- senior speech and language therapist
- physiotherapy technical instructor
The team provides specialist assessment, rehabilitation care and advice to people for a range of problems including difficulties in speaking, swallowing, moving and doing day to day activities like making a cup of tea, or brushing their teeth. The team also helps people who are prone to falls or who have a neurological condition such as Alzheimers.
The older person support team works closely with a variety of other health care teams within the community and also at hospitals. The teams may include, specialist doctors, specialist nurses, physiotherapists, psychologists, dieticians and occupational therapists.
Working together we discuss patients needs and decide how best to manage their treatment and care.How can I access the service?
Patients cannot refer themselves to the service.
The older person support team's physiotherapist gets referrals for patients who are temporarily placed into short stay nursing beds funded by North Tyneside Primary Care Trust. We also get referrals from North Tyneside's community matrons and emergency care practitioners.
The speech and language therapist from the older person support team will accept referrals from all healthcare professionals.
The team are also involved in providing follow up visits to patients once they leave hospital in a number of areas including stroke care.
