Community learning disabilities

The community learning disabilities team provides a range of services for people with a learning disability. The services we offer include:

  • Community nursing
  • Assessment and care co-ordination
  • Behaviour assessment and treatment
  • Psychology
  • Psychiatry
  • Speech therapy
  • Independent supported living
  • Support for people with autism
  • Support for parents and families
  • Care management
  • Mental health
  • Forensics
  • Epilepsy care
  • Support for adults with learning disabilities and complex health needs

The services are provided by nurses, social workers and other professionals who work both in the home and in primary care settings such as clinics and GP surgeries. Social care services are offered in line with guidance and criteria in relation to Fair Access to Care.

How can I access the service?

Referrals are accepted from:

  • carers
  • parents
  • GPs
  • nursing staff
  • social workers
  • self referral

Information is required regarding confirmation of a person having a learning disability to ensure the appropriate services are offered. Referrals can be made by letter, in person, telephone or by using an Easy Care referral form. Please ask your GP or contact us at the address below for further details.
