About the service
North Tyneside Primary Care Trust's (PCT) community cardiology team provide a range of services for patients suffering from coronary heart disease.
The services the team provide includes:
- Cardiac rehabilitation programmes - the aim of cardiac rehabilitation is to help you to recover and resume as full a life as quickly as possible after a cardiac event such as a heart attack or heart surgery. - Heart manual visits
- Home visits both before and after coronary artery bypass grafts
- Home visits after valve surgery
- Home visits both before and after percutaneous coronary intervention (angioplasty)
- Care for patients with heart failure
- Improved communication between primary, secondary and tertiary care (in hospitals)
- Primary and secondary prevention –
- primary intervention involves encouraging people to improve their health and reduce their cardiovascular risk by giving up smoking, maintaining a healthy diet and doing more exercise before becoming ill, i.e. before a heart attack.
- secondary prevention is encouraging all the above following a heart attack.
- Joint working with health partnerships/public health/local council/Stroke Northumbria.
The community cardiology team also supports other primary care colleagues in treating patients with heart disease. This support is provided through the circulation of up-to-date guidelines and the provision of education and training sessions.
How can I access the service?
Patients are referred into cardiac rehabilitation programmes following a heart attack, angioplasty/stent or heart surgery via their hospital doctor or GP. Patients access the community matron for heart failure services via their GP, consultant cardiologist or primary health care team, i.e. district nurse.
Two cardiac rehabilitation programmes are running for patients in North Tyneside, each consisting of two sessions per week. Patients referred for cardiac rehabilitation will be allocated to a six week programme at either:
The Victoria Therapy centre, Hawkeys Lane, North Shields.
Wednesday: 1pm – 4pm and Friday: 1pm- 3.15pm

The Parks Leisure centre, Royal Quays, North Shields.
Tuesday 1pm - 4pm and Thursday 1pm - 3.15pm
